YSA CONFERENCE PARTICIPATION AGREEMENT
Welcome! We are excited to have you participate in the Utah Area YSA Conference (the “Event”). Participation in the Event is a privilege that can be revoked, so please carefully read this YSA Participation Agreement (this “Agreement”). This Agreement is a binding contract between you (referred to as “you” or “your”) and The Church of Jesus Christ of Latter-day Saints, a Utah corporation sole (referred to as “we”, “us”, or “our”). Do not register for the Event unless you agree to all the terms of this Agreement. By registering, you agree to all the terms of this Agreement.
YSA Participant’s Commitment
You are responsible for your conduct at the Event. In this regard, you will follow the standards of The Church of Jesus Christ of Latter-day Saints (the “Church”) while at the Event. You will follow all rules and policies of the Event; those given as part of your registration for the Event or during the Event; and the principles taught regarding dress and appearance in the Church’s “For the Strength of Youth: A Guide for Making Choices” (pp. 24-27), all as may be updated from time to time (collectively, “Policies”). Your conduct and interactions at the Event should strive to reflect Christlike behavior. We make decisions about your participation at the Event based on these Policies. You will honor instructions of the employees, volunteers, affiliated entities, employees of affiliated entities, and other agents and third parties that we retain to help operate the Event (“Event Personnel”) regarding dress, standards of conduct, and all other Policies, and to stay informed of these Policies as they may be updated.
If we determine that you fail to keep any of these Policies or that your words or actions pose a risk to you or others or to the fundamental purposes of the Event, we may excuse you from the Event, with no refund. Indeed, we retain the right (without penalty and at our discretion) to exclude anyone from the Event for any or no reason.
Prohibited Items
The following items are not allowed at the Event or its premises: illegal items, dangerous items (including any weapons), alcohol, or controlled substances (other than those duly prescribed by a medical professional). You consent to a reasonable inspection of your person and belongings for prohibited items while on the premises associated with the Event.
Consent to Medical Treatment
You affirm that you can safely participate in the Event. You consent to receive any medical treatment and any related medical transportation deemed necessary by any medical personnel, first responders, or Event Personnel if you are injured or require medical attention during your participation in the Event. You are solely responsible for all costs related to such medical treatment and transportation (if necessary), including transportation to a hospital. You release and hold us harmless, as well as any medical personnel, first responders, and Event Personnel, from any claim based on such treatment or medical services (whether provided or withheld).
Assumption of Risk and Waiver of Liability
You acknowledge that you are voluntarily participating in the Event and agree to accept and assume any and all risks of injury, disability, death, and property damage arising from your participation in the Event, whether caused by our ordinary negligence, a third-party provider, law enforcement personnel, other first responders, Event Personnel, or others. Additionally, you waive any claim you may have against us or any Event Personnel, and release each from any liability arising from your participation in the Event. You are solely liable for any damage or harm you cause to yourself, others, us, or any Event Personnel.
Privacy
We may collect and process personal information obtained from your registration and Church membership records to facilitate participation in the Event and to respond to questions or issues that may arise. We will handle your personal information in accordance with our Privacy Notice.
Personal Information
If you are invited to participate in sharing information about yourself (for example, personal or other information) in connection with the Event through a third-party application or provider, you acknowledge that all such participation is entirely voluntary and is done at your own risk. You have no obligation to participate. You further acknowledge that you assume all risk associated with such participation and release us and Event Personnel from any and all liability.
Research Studies and Surveys
If you are asked to participate in a study or survey, any such participation is entirely voluntary, and you may withdraw from participating at any time. Any contact information provided as part of a study or survey (for example, email address or mobile phone number) will be used only to contact you about participating in future studies or surveys or to follow up regarding a study or survey in which you have already participated.
Consent to Use of Name and Image
We may film, photograph, or record you, other participants at the Event, and Event Personnel, either individually or as part of a group, in connection with the Event. You authorize us to film, videotape, photograph, or otherwise record you, including your name, voice, image, likeness, story, and performance (collectively referred to as your “Name and Likeness”) in connection with the Event. You authorize us, Event Personnel, and our affiliated entities to copy, reproduce, distribute, broadcast, publish, adapt, edit, display, perform, translate, summarize, transmit, create derivative works from, and otherwise use and allow others to use your Name and Likeness in any and all manners, in any language, anywhere in the world, and in any and all manners and forms of media as long as such use is consistent with our Privacy Notice. You release us from all liability from such recording and use. You also waive all rights of publicity and rights of privacy with respect to use of your Name and Likeness in accordance with this Agreement. You waive any right to inspect, approve, or be compensated for the recording or use of your Name and Likeness in accordance with this Agreement. We are not required to use your Name and Likeness in any way.
Submission Release
If you text, email, physically deliver, post, submit, or otherwise directly or indirectly provide any photograph, recording, video, writing, comment, drawing, artwork, story, or other creative material that you created (or helped create) to us or any Event Personnel in connection with the Event (“Submissions”), then we can (without any compensation to you) use such Submissions in any and all manners consistent with our Privacy Notice. You grant us an unlimited, non-exclusive, worldwide, royalty-free, perpetual, and irrevocable license to copy, reproduce, distribute, broadcast, publish, display, perform, transmit, translate, edit, adapt, create derivative works from, and to otherwise use (and allow others to use) your Submissions in any and all languages, formats, and manners. You agree that any use of your Submissions in accordance with this Agreement will not violate your rights. Moreover, you agree that you have all rights, licenses, releases, waivers, and permissions that are needed to grant the rights and licenses set forth in this Agreement. With respect to us and any anyone who receives rights in your Submissions from us, you also agree, to the fullest extent permitted by applicable law, to waive and to not assert any so-called "moral rights" relating to use of your Submissions.
General
We engage Event Personnel and other third parties to help administer and promote the Event. Any such third party is an intended beneficiary of this Agreement and may have the right to enforce and receive the benefit of certain terms of this Agreement. This Agreement constitutes the sole and entire Agreement between us with respect to the subject matter in this Agreement and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. Except with respect to any provision determined by us to be significant or essential, if a court holds any term or provision of this Agreement to be invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability will not affect any other term or provision of this Agreement or invalidate or render unenforceable such term or provision in any other jurisdiction. For clauses we determine in our sole discretion as being significant or essential, you and we will mutually agree in writing on a replacement provision. If a replacement provision cannot be agreed upon, we may terminate this Agreement immediately upon written notice. This Agreement is binding on and will inure to the benefit of the parties and their respective successors and assigns. This Agreement is governed by the laws of the State of Utah, USA, without giving effect to principles of conflicts of law. Any legal action relating to this Agreement or to the Event must be brought in the state or federal courts located in Salt Lake County, Utah, USA, and you consent and submit to the personal jurisdiction of those courts.

YSA CONFERENCE TICKET AGREEMENT
By either attending the Utah Area YSA Conference (the “Event”) or by clicking the “Accept” button, you agree to the terms of the following agreement between you and The Church of Jesus Christ of Latter-day Saints, a Utah corporation sole (the “Church”). If you disagree with any of these terms, do not attend the Event.
REFUNDS—All claims relating to the Event are limited to a refund of the ticket price (if any) to the Event. Service charges are not refundable.
TICKETS—To attend the Event, you must have a ticket. Unauthorized sale or duplication of a ticket may result in your being denied admittance to the Event. The Church is not responsible for any inconvenience caused by an unauthorized ticket sale or duplication. If duplicate copies of a ticket are presented at the Event, the Church reserves the right to deny entry to the Event to any and all parties.
CONDITIONS OF USE—The Church retains the right (without penalty and at its discretion) to exclude anyone from the Event for any or no reason (including for posing a risk or being disruptive). Any activity likely to interfere with the Event or endanger you or others may result in immediate eviction from the Event and its premises.
PROHIBITED ITEMS—The following items are not allowed at the Event or its premises: illegal items, dangerous items (including any weapons), alcohol, or controlled substances (other than those duly prescribed by a medical professional). You consent to a reasonable inspection of your person and belongings for prohibited items while on the premises associated with the Event.
ACCESSIBILITY AND AMERICAN SIGN LANGUAGE (ASL)—Accessibility requests (including, but not limited, to ASL interpretation) must be made at least 30 days in advance of the Event. We cannot guarantee that all such requests will be met.
PERMISSION TO BE RECORDED—You consent to the Church (and anyone it authorizes): (1) taking audio and visual recordings and photos at the Event (“Recordings”) of you and of your voice, likeness, and image and (2) using such Recordings in broadcasts of the Event and in any and all creations that the Church (and anyone it authorizes) creates with the Recordings.
PERSONAL INFORMATION—If you are invited to participate in sharing information about yourself (for example, personal or other information) in connection with the Event through a third-party application or provider, you acknowledge that all such participation is entirely voluntary and is done at your own risk. You have no obligation to participate. You further acknowledge that you assume all risk associated with such participation and release the Church from any and all liability.
